1. Are You Eligible?
Patients must meet the following qualifications to be seen at the Clinic:
- Newborn to 65 years of age (patients under 18 must be accompanied by a parent or legal guardian)
- Permanent resident of northern Pinellas County
- U.S. Citizen
- No medical insurance or government medical assistance
- Must meet financial guidelines
2. Read the Patient Guidelines
We expect all Clinic patients to read and follow our Patient Guidelines. Failure to follow these guidelines may result in dismissal as a Clinic patient.
The Clearwater Free Clinic is an independent non-profit, non-government medical facility that is staffed primarily by dedicated volunteers. Clinic services are made available by volunteers, donated services, and contributions from patients, private individuals, businesses, grants, and fundraising. The Clinic provides access to excellent health care that might otherwise not be available to you.
3. Make Your Appointment
Call (727) 447-3041 or visit the Clinic in person to make an appointment. You must make your appointment on the same day that you wish to be seen.
If you are unable to keep your appointment, you must call to cancel a reasonable time before your appointment so that we may use the time to see other patients who are waiting. If you do not cancel, you may risk not being seen in the future.
4. When you come

Please arrive 30 minutes before your scheduled appointment to fill out paperwork.
Bring the following documents with you to your first visit:
- Proof of U.S. citizenship: birth certificate, voter ID card (not voter registration), or U.S. passport
- Photo ID (driver's license or state ID card)
- Social Security card
- Proof of residence in upper Pinellas County (rent receipt, utility bill, phone bill, etc.)
- If the patient is a child under the age of 18, they must be accompanied by a parent or guardian and you must bring:
- Child's birth certificate with parents' names listed
- Child's Social Security card
- Parent or guardian's photo ID (driver's license or state ID card)
- Parent or guardian's Social Security card
- Proof of residence (rent receipt, utility bill, phone bill, etc.)
Be prepared to make a monetary donation at the time of your visit. Although there is no set fee for Clinic services, we depend on donations from our patients to keep the doors open!
