HOW DO I BECOME A PATIENT OF THE CLINIC?
All new patients must call 727-331-8148 to be pre-approved during the following hours:
Monday -Thursday**
8:30 am – 5:00 pm
**New Patient Registration is CLOSED for lunch 12:00 pm - 1:00 pm every day.
ADULT PATIENT ELIGIBILITY REQUIREMENTS – 18-64 YRS
TO BE ELIGIBLE, YOU MUST:
Be a resident of mid or upper Pinellas County (North of Park Blvd)
Be a US citizen or Permanent Resident of the US
Meet low-income financial guidelines
Not have any insurance, including Medicare/Medicaid, County Social Services, VA Benefits
ADULT PATIENTS 18 – 64 YEARS OF AGE
Proof of US Citizenship or lawful permanent residence: Please bring one of the following
US Birth Certificate
US Passport
Voters ID
US Naturalization Certificate
Permanent Resident Alien Card (I-551)
Proof of Current Address: Example: rent receipt, bank statement, cable or utility bill, any mail addressed to you
Florida Drivers License or State ID Card
Social Security Card
PEDIATRIC PATIENT ELIGIBILITY REQUIREMENTS – INFANTS TO 17 YEARS OF AGE
TO BE ELIGIBLE, YOU MUST:
Be a resident of mid or upper Pinellas County (parents must provide documentation of address)
Be a US citizen or lawful permanent resident
Meet low-income financial guidelines
Not have any insurance, including Medicaid, County Social Services, KidCare
Please bring all 4 required documents during pre-approval hours (listed below).
Proof of Child’s US Citizenship or lawful permanent residence: Please bring one of the following:
US Birth Certificate – Parent must be listed on birth certificate
US Passport
US Naturalization Certificate
Permanent Resident Alien Card (I-551)
Foreign Passport stamped by the U.S. Government “Processed for I-551”
Parent’s Proof Current Address: Example: rent receipt, bank statement, cable or utility bill, junk mail addressed to you
Child’s Social Security Card
Parent’s Photo ID
NOTE:
Current tax information may be required for medication assistance and income verification
DONATIONS
Patients should come prepared to make a donation at each visit to the Clinic. Medical services are donated by our professional volunteers, however, your donations are needed to help purchase medications, medical supplies and offset operating expenses necessary to keep the doors open and the lights on. Every donation will help ensure that the Clearwater Free Clinic is here for your future needs.
The Clearwater Free Clinic does not provide physical exams or office visits for those who are involved in litigation.
